Pharmacy Students- UGA OneCard ($30) and Pharmacy nametag ($10). Please email headshot photo to ugacard@uga.edu & include your name and Order Number.

DO NOT CHOOSE SHIPPING. Your ID and badge will be issued to you at your Orientation. They will NOT be mailed.

Standard $40 package includes your Student ID card and your clip-on nametag. If you already have an ID with the "LEAF" designation on the back of the card, you only need to order the $10 name tag. You can choose your options under "Request Reason".

Eligibility Statement-I certify that I am, or will be, a student enrolled in the Pharmacy program at the University of Georgia. Giving false or incomplete information in the completion of this form may be considered a falsification of UGA records and disciplinary action shall be taken. I understand that NO REFUND will be made for any reason.
