MSAT Students- Athletic Training nametag ($10). We will use the same photo you uploaded to order your UGA OneCard (student ID).

Select Campus Delivery option. Name tag and ID will not be mailed. Jill Manners will pick up all IDs and you'll get them from her when your program begins.

Eligibility Statement- By completing the order process, I certify that I am, or will be, a student enrolled in the MSAT program at the University of Georgia. Giving false or incomplete information in the completion of this form may be considered a falsification of University records and disciplinary action shall be taken. I understand that NO REFUND will be made for any reason.
